During Your First Visit, We Will:

After we finish these important steps, the doctor will meet with you to discuss your X-rays, help you create a personalized treatment plan, answer any of your questions, and address any remaining concerns.

If you are a new patient, please download the forms linked below and fill them out before your first appointment. Then, please bring the printed copies with you as well as your insurance card.
